REGISTRATION FORMS

Forms are available online or at Fin and Feather Sports in Upton.Please download the registration form PDF to print out. Fill out both sides and mail or drop it off to the Fin and Feather Sports with check or money order made out to: “BV FISHING DERBY”

Mail it to: Fin and Feather Sports, PO BOX 314 Upton, MA 01568 by April 25th.

Three items must be completed:

1. Please make check payable to: BVCC Fishing Derby. Questions, call Jeff at 508-529-3901.

2. Download the form now to print off, mail or drop off.

3. Waiver of Liability Form must also be downloaded and signed.

All that pre-register by APRIL 25TH
will receive an Official Youth Derby T-Shirt.
