REGISTRATION FORMS
Forms are available: to download online or at
Fin and Feather Sports in Upton or at the
Blackstone Valley Chamber of Commerce Office in
Northbridge. Please download the
registration form a PDF to print out. Fill out
both sheets and mail or drop it off to the Fin
and Feather Sports with check or money order
made out to: “BV FISHING DERBY”
Mail it to: Fin and Feather
Sports, PO BOX 314 Upton, MA 01568 by May 7,
2011 for a free official derby T-shirt and
to be entered into the drawing for a Free Summer
Clinic (4 day session) at Fin & Feather Sports
/ Outdoor adventure program this Summer. *
Drawing to be held at the Fishing Derby awards
ceremony.
Three items must
be completed:

Please make check payable to: BVCC Fishing
Derby.
2.
Please download
the
Registration Form
now to print off, mail or drop off.
3.
Please download
the
Waiver
of Liability Form
which must also be signed.
